• Official Visa Invitation Letter (VIL) A formal letter issued by an accredited partner hospital in Delhi, India, stating your medical history, proposed treatment, and duration of stay. Mustasfaa coordinates this letter for you.
• Recent Diagnostic Reports & Scans Carry physical and digital copies of your MRI, CT scans, blood counts, and local doctor referral notes.
• Passport Size Photographs 2 recent white-background photos (size 2 inch x 2 inch).
• Local Doctor Referral Letter (Bangladesh) Indian Embassy in Dhaka requires a written reference letter from a registered medical practitioner in Bangladesh confirming you require treatment abroad.
• Utility Bill & Active Bank Statement Copy of current electricity/gas bill in Bangladesh and bank statements showing sufficient funds for travel.
• Yellow Fever Vaccination Card Mandatory for all arrivals from Africa. Card must be issued at least 10 days before departure.
• Oral Polio Vaccine (OPV) Certificate Indian immigration requires OPV certificate issued at least 4 weeks prior to entry for travelers from Kenya and Nigeria.
• National ID & Medical Syndicate Approval (Iraq) Original Iraqi civil status card (National ID) and registration details matching the Arabic/English passport translation.
• Ministry of Health Referral (Oman) If sponsored by the government of Oman, carry the ministry decision copy for direct billing validation at Delhi hospitals.
• Exit Visa Clearance & Sponsor Details (Uzbekistan) Check passport biometric pages for exit sticker validation. Carry translated financial guarantee letters.
